Below are forms necessary to enroll your child in Anointed Christian Childcare as well as the forms necessary to keep your child's/children's records updated with us.

You will need Adobe Acrobat to print the forms.  Once you print them, just fill them out and bring them into Anointed Christian Childcare when you come in to register your child. 

If you need to make changes at anytime, just fill out the appropriate new forms and bring them in to us.  That's it!!

ENROLLMENT REQUIREMENTS

(ALL ITEMS ARE REQUIRED PRIOR TO CHILD RECEIVING CARE.)



1. Current shot records for all children

2. Driver's License & Social Security Card for contracting parents

3. Parent's employment verification Form

4. First week's payment + equal deposit (2 week's payment + equal deposit ) for bi-weekly clients.

5. Completed mandatory paperwork
